Form with document view and category data side by side?

Hi,

We're trying to build something like the picture under 'powerful forms' on the webreports overview page: http://www.resonatekt.com/products/webreports#null

Are there samples of how to build something like this?

We would like to display a document with metadata, and have the user select a folder to copy the document to.

Optionally, we would also like be able to use a solution like this as a step in a workflow.

Would this be possible using WebReports?
